Stop Trying to Lower Your Thyroid Antibodies: Why TPO Isn't the Enemy in Hashimoto's

If you have Hashimoto's, you already know the drill. You get your blood drawn, you refresh the patient portal, and you scan straight for two numbers: your TSH and your TPO antibodies.
Maybe your TSH finally landed in range. Good. But those thyroid antibodies are still high, and now every ounce of your attention goes toward getting that number down.
If you're on the Hashimoto's side of social media, you've seen why. Practitioners post case after case: someone walked in with antibodies five times the reference range, did a protocol, and now their antibodies are "normal." It gets celebrated like a cure. What almost nobody mentions is whether that person's symptoms actually resolved.
Because here's what I see constantly in my own practice: a normal TSH and even normal antibodies, and the person is still exhausted, still losing hair, still foggy, still stuck at the same weight no matter what they cut out or how hard they train.
Today I want to walk you through why lowering TPO antibodies isn't actually the goal for most people with Hashimoto's, and what the real target is instead. This is Florida-based functional medicine built for people managing real symptoms, not just a lab printout — and whether you're reading this from down the street or from another country entirely, the physiology underneath your labs works the same way.
Too Simple — Often Overlooked: Are You Taking Your Thyroid Meds Right?
Before we get into the immunology, check this first: are you taking your thyroid medication 30 minutes before food or coffee?
Thyroid hormone in pill form is notoriously hard to absorb. Under ideal conditions you absorb about 80% of what you swallow — worst case, as low as 40%. Coffee, breakfast, or a multivitamin with calcium or iron within that 30-minute window drops it further, because food changes your stomach pH and calcium, iron, and the polyphenols in coffee all bind directly to the medication.
I've had patients come to me convinced they had "treatment-resistant Hashimoto's" because their TSH never moved no matter the dose. The entire problem was that they got up, took their pill, and had coffee ten minutes later. We fixed the timing, not the immunology, and their labs and their symptoms both improved. Check the simple things before you go chasing advanced immunology.
Core Concept 1: Why Aren't Your TPO Antibodies the Enemy?
Because they don't destroy your thyroid tissue — they just detect the problem.
There are two thyroid antibodies in Hashimoto's: TPO (thyroid peroxidase, present in about 95% of Hashimoto's cases) and ATA or antithyroglobulin antibody (present in only 20–50%, depending on the study). Both target proteins involved in making T3 and T4. That's why almost everyone talks about TPO specifically — nearly everyone with Hashimoto's has it.
But here's the disconnect: people assume that because higher TPO numbers mean worse Hashimoto's, lower numbers must mean better Hashimoto's. That's not how it works. Think of TPO antibodies as a smoke alarm in your kitchen. If there's a kitchen fire, the alarm didn't start it and can't put it out — it just screams that something is burning. The actual fire, the thing physically destroying your thyroid cell by cell, is being carried out by your cytotoxic T cells and a cascade of inflammatory cytokines. You can rip the alarm off the wall and pull the battery, but the kitchen can still be on fire.
In practice, I regularly see people get worse symptomatically while their antibodies go down, and get better while their antibodies go up. I can work with a patient who is stable and fully functional with antibodies in the 2,000s — 50 times the reference range — while someone else is falling apart with antibodies barely above normal. Let me say that part again, because it's the one thing I want you to walk away with: your antibody level is not tied to how severe your Hashimoto's actually is.
Core Concept 2: What Are the 4 Stages of Hashimoto's?
Hashimoto's, like every autoimmune condition, develops in stages — and it behaves like a slow leak in your roof.
- Stage 1 — Silent Hashimoto's: the leak nobody's noticed yet. You have thyroid antibodies on a blood test, but no symptoms and a normal thyroid panel.
- Stage 2 — Hashimoto's reactivity: the first wet spot on the ceiling. You're symptomatic — brain fog, fatigue, needing more sleep — but your blood work still hasn't shifted.
- Stage 3 — Lab-positive hypothyroidism: water is actively dripping through. Enough thyroid tissue has been destroyed that your TSH rises and you need medication.
- Stage 4 — Established hypothyroidism with unresolved autoimmunity: the roof caves in. You're medicated and your TSH may be normal, but the underlying autoimmune process was never addressed.
This matters because, by most clinical estimates, roughly half of people who go on thyroid medication and normalize their TSH still have unresolved symptoms. Medication fixes your hormone levels. It does not fix the fact that you have Hashimoto's.
Core Concept 3: Could Your TPO Antibodies Be Affecting Your Brain?
In the specific case of cross-reactivity, yes — and this is the one confirmed exception to "don't chase your antibody number."
Cross-reactivity, or molecular mimicry, happens when two proteins look similar enough that your immune system can't tell them apart, so it attacks both. A foundational 2007 study in the Journal of Neuroimmunology showed that TPO antibodies can cross-react with astrocytes in your cerebellum — the part of your brain responsible for calibrating movement, balance, and, less obviously, emotional and cognitive control.
If you have Hashimoto's and you're dealing with dizziness, clumsiness, unsteadiness, or vertigo, your immune system may be attacking your cerebellum right alongside your thyroid. And because the cerebellum also helps rein in negative thinking, this can show up as anxiety or emotional dysregulation that feels disconnected from any obvious stressor. Two more cross-reactions — with the antimicrobial enzymes MPO and LPO — are theoretically sound but not yet confirmed in humans; they'd show up as trouble fighting bacterial infections or, in women, unexplained breast pain.
This is the one scenario where actively working to lower your TPO antibodies deserves real attention. Everyone else: keep reading.
Core Concept 4: Does Selenium Actually Lower TPO Antibodies?
Only if you're already deficient — and even then, it takes at least six months.
Selenium is on almost every Hashimoto's supplement list you'll find online. The research behind it is real, but it comes with fine print most influencers skip:

The mechanism is real: selenomethionine supports glutathione production inside your thyroid cells, which protects them from damage and has a balancing effect on your immune system. But without an actual deficiency, you're just spending money. Before you buy a bottle, get your selenium level checked and check your balance and coordination — if both are normal, don't bother.
The Final Word: How Do You Actually Build Immune Competence?
Not by chasing a lab number. By working through your fundamentals, in order, every time. Here's the restoration framework I use with my own patients:
- Fix your diet. Don't eat foods you react to — every reaction stirs your immune system. Go fully gluten-free, not "mostly" (research ties gluten cross-reactivity to thyroid tissue, especially in celiac disease). Watch lectins, particularly in red kidney beans, which have been shown to cross-react with thyroid tissue. And stabilize your blood sugar — every glucose spike or crash is its own inflammatory event.
- Protect your sleep. This is when your brain clears metabolic waste and your immune system does its repair work. Chronic poor sleep functions the same as chronic low-grade inflammation.
- Build real community. Isolation drives up stress chemistry and inflammation. Genuine social connection turns it back down.
- Clean up your environment. Ditch plastic water bottles and to-go cups for glass or stainless steel, especially with hot or acidic liquids — heat and acidity leach plastics straight into what you're drinking. Look hard at personal-care and cleaning products, since many carry a real endocrine-disrupting load.
- Get proactive, not reactive. This is about establishing immune competence and control across the whole picture — not one supplement chasing one number.
Your antibodies are almost never the enemy. They're the smoke alarm, not the fire. If you've been doing everything you were told to do and you still don't feel right, that's not a personal failing — it's a sign you need a more complete picture than any podcast or Instagram reel can give you.
