Does Gluten Really Cause Leaky Gut in Everyone? Allergy vs. Sensitivity vs. Intolerance (Part 1)

Last November, one of the biggest medical journals on the planet published a review that a lot of people in the gluten-free world did not want to hear. Most people who believe they're reacting to gluten, in properly controlled trials, react to gluten about the same as they react to a placebo.

Before you throw out your gluten-free bread, stick with me. That same body of research, and a stack of other studies, also confirms that plenty of gluten reactivity is completely real. It's measurable. It's mechanistic. And in some cases it's tangled up with your gut lining, your thyroid, and even blood flow to your brain. The honest answer isn't "gluten is poison," and it isn't "gluten sensitivity is fake." It's messier than either extreme.

I get asked about gluten more than almost any other food-related topic in my practice. I have patients convinced gluten is slowly destroying them, and I have patients convinced cutting it out was a waste of a perfectly good donut. Sometimes both of those people are right. Sometimes both are wrong. The only way to actually know which one you are is to understand what's happening in your body when you eat a piece of bread — not what an influencer or an elimination diet told you should happen.

This is Part 1 of a two-part series. Today we're defining what gluten actually is, walking through the two gut reactions that happen in everyone who eats wheat — sensitive or not — and untangling three terms most people use interchangeably: allergy, sensitivity, and intolerance. Part 2 covers celiac disease vs. non-celiac gluten sensitivity, gluten's contested relationship with your thyroid, and how gluten affects blood flow to your brain.

TOO SIMPLE — OFTEN OVERLOOKED: What Is Gluten, Actually?

Gluten isn't one thing. It's a family of storage proteins found in wheat, barley, and rye. About 70% of the proteins in wheat fall into the gluten category, and that category splits further into two subclasses: gliadin and glutenin.

When you mix flour with water and knead it, gliadin and glutenin link up into a stretchy, elastic network. That network traps gas bubbles from yeast and gives bread its chew and structure. No gluten, no stretch — which is exactly why old-school gluten-free bread didn't rise, didn't stretch, and had a mouthfeel nobody asked for. Gluten-free baking is its own science project because you're rebuilding scaffolding that gluten normally supplies, with completely different materials.

Gliadin itself splits further into alpha, gamma, and omega gliadin. For decades, medical research fixated almost entirely on one piece of that puzzle — the alpha-gliadin fragment tied to celiac disease — while gamma-gliadin, omega-gliadin, and glutenin were largely ignored as potential causes of wheat-related complaints. That narrow focus is exactly why so many conventional doctors still operate on one rule: if you don't have celiac disease, gluten isn't your problem. The research doesn't actually support that rule, and it hasn't for years — it just takes time to trickle down. Information moves from research to clinical practice at roughly a 17-year lag on average, and non-celiac gluten sensitivity wasn't even formally named until 2011, thirty years after scientists had already worked out gliadin's different subtypes.

Does Gluten Cause Leaky Gut in Everyone?

Yes — and this is not a per-person experience. It's a known reaction that happens whether or not you're gluten sensitive at all.

Here's the mechanism. The gliadin protein binds a receptor in your gut lining called CXCR3, which triggers the release of a protein called zonulin. Zonulin opens the gut barrier — like pushing on a turnstile to get through the subway. As zonulin levels climb, you get a greater degree of leaky gut, and the more frequently you eat gluten, the longer that door stays propped open, because you're constantly producing more zonulin to push it back open again.

Let me say that clearly, because it surprises people: this connection between gluten and leaky gut does not require you to be gluten sensitive. No matter who you are, when you eat gluten, you get a transient leaky gut. That's universal. The word doing the work there is "transient" — in someone without celiac disease, that door opens briefly and closes back up. In someone with celiac disease, the underlying autoimmune process keeps hammering on it, and it doesn't close the same way.

What Do ATIs Do to Your Gut Even If You're Not Gluten Sensitive?

They trigger inflammation — a second universal reaction that, like the leaky gut mechanism above, doesn't depend on whether you're gluten sensitive at all.

This one comes from the non-gluten side of wheat. Alongside proteins like serpins and purinins — which some people react to and some don't — wheat contains a family of proteins called amylase trypsin inhibitors, or ATIs for short. ATIs bind to a receptor called TLR4 (toll-like receptor 4) on cells of your innate immune system — the same receptor, and the same chemical response, your immune system uses when it reacts to LPS, a toxin produced by bad bacteria in your gut.

The net effect is threefold: ATIs trigger their own inflammation, they amplify any pre-existing inflammation already sitting in your gut, and they remodel your microbiome in an unfavorable direction. And yes — the same toll-like receptors ATIs bind to also promote the transient leaky gut we just covered. So when you eat wheat, even if you're not gluten-sensitive by any definition, you can count on two universal responses: a transient leaky gut, and a greater inflammatory load in your gut that can go systemic.

Is It a Wheat Allergy or a Gluten Sensitivity? (They're Not the Same Thing)

A wheat allergy and a gluten sensitivity are not the same disease wearing two different names. They're two different branches of your immune system doing two different jobs.

A true wheat allergy is IgE-mediated. Your immune system flags a wheat protein as dangerous, arms mast cells with IgE antibodies specific to it, and the next time wheat shows up, those mast cells dump histamine within minutes — hives, itching, swelling, wheezing, and in the worst cases, anaphylaxis. It's fast, it's dramatic, and it's confirmed with a skin prick test or an IgE blood panel that either lights up or it doesn't.

Gluten sensitivity runs through a different branch entirely — the IgG side of the immune system — and it looks nothing like an allergy. There's no mast cell explosion, no massive histamine dump, and no single blood marker that lights up the way an IgE panel does. What you get instead is a slower, murkier response: bloating, fatigue, brain fog, or joint pain, often showing up hours later instead of minutes. And where allergy symptoms are reproducible and identical from person to person, sensitivity symptoms are person-dependent — one person gets gut symptoms, another gets brain fog, a third gets joint pain, all from the same trigger.

Think of it like the difference between a smoke alarm and a slow gas leak. The smoke alarm — the allergy — goes off the instant there's a problem. Loud, unmistakable, everyone in the house knows exactly what triggered it. The gas leak — the sensitivity — has no alarm at all. You just start feeling a little worse every day, and it takes real investigation to find where it's coming from, because nothing obvious set it off.

Wait — Isn't Gluten Intolerance the Same as Sensitivity?

No, and this is where the terminology gets genuinely misused. An intolerance isn't an immune response at all — it's an enzyme deficiency.

The classic example is lactose intolerance: someone lacks the enzyme lactase, which breaks down the sugar lactose in dairy. No lactase, no breakdown — and when they eat dairy, they're in for bloating, distention, and osmotic diarrhea. That's a plumbing problem, not an immune reaction. So when someone says "I'm gluten intolerant" and means the bloating-and-fatigue picture we described above, that's the wrong word for what's actually happening — that's a sensitivity, an IgG-branch immune response, not a missing enzyme.

Let me say it one more time, plainly: an intolerance is not an allergy, and it's not a sensitivity. Sensitivity and allergy are both immune responses, just on two different branches. Intolerance isn't an immune response at all.

THE FINAL WORD: Where This Leaves You (And What's Coming in Part 2)

Here's what's true no matter who you are: gluten gives you a transient leaky gut, and wheat's ATI proteins add their own inflammatory load — both universal, both independent of whether you're "gluten sensitive." On top of that universal baseline sits a second layer that is entirely person-dependent: whether you have a true wheat allergy (IgE, fast, reproducible), a gluten sensitivity (IgG, slow, individual), both, or neither.

Everyone who has celiac disease is gluten-sensitive. Not everyone who is gluten-sensitive has celiac disease.
