ATIs, Celiac Disease, and Gluten's Cross-Reaction With Your Thyroid (Part 2)

Last episode I covered what gluten actually is and the two things that happen in every single person's gut when they eat it. This episode is Part 2, and it's where things get interesting: the other wheat protein doing damage, where celiac disease actually ends and a murkier sensitivity picture begins, what the evidence really says about gluten and blood flow to your brain, and the question I get asked more than almost any other in my practice — does gluten cross-react with your thyroid?

If you're on the gluten-free side of social media, you've probably heard some version of "gluten destroys your thyroid" delivered as settled fact in a 90-second clip. And if you're on the other side, you've probably heard a dietitian tell you wheat is an essential part of a healthy diet and cutting it will wreck your nutrition. Both of those are wrong, or at least badly oversimplified. Poppycock, honestly. There's no nutrient in wheat you can't get somewhere else, and the real story here is more interesting — and more useful — than either extreme.

What Is Amylase Trypsin Inhibitor, and Why Does It Matter as Much as Gluten Itself?

Amylase trypsin inhibitor — ATI for short — is a non-gluten wheat protein, and it does something gliadin doesn't need your immune system's help to do. ATIs bind directly to a receptor called toll-like receptor 4 on your innate immune cells — the same receptor bacterial toxins use to set off inflammation. That single binding event does three things you don't want: it opens your gut lining and creates its own transient leaky gut, it fires off fresh inflammation from cells like neutrophils and macrophages, and it amplifies whatever inflammation is already sitting in your gut.

Here's the part that should actually change how you think about "whole grain" as a health food. A 2020 study found that ATIs remodel your gut microbiome the same way a course of antibiotics does — suppressing anti-inflammatory bacterial species and letting pro-inflammatory ones take over. Think of ATIs as a low dose of antibiotics you never got a prescription for, refilled every time you eat a sandwich. And it gets worse: some of the bacterial toxins called lipopolysaccharides, or LPS, that live in the outer shell of grains bind directly to ATI proteins, hitching a ride into your gut every time you eat them. LPS and ATIs both run highest in whole grains, brown rice included — not white rice, brown rice — because that's where the outer shell, and the bacteria living on it, actually is.

All grains carry some level of ATI and LPS. That's a big part of why so many people who go fully grain-free on a paleo-style diet feel dramatically better — they've removed a genuine, measurable inflammatory load, not just cut a few calories.

Where's the Actual Line Between Celiac Disease and Non-Celiac Wheat Sensitivity?

Most people, and most conventional doctors, treat a gluten problem like an on-off switch — you either have celiac disease or you're fine. That's not how it works. It's a spectrum, and celiac disease is only the single most extreme, most clearly defined point on it.

Celiac disease is autoimmune. In someone with the HLA-DQ2 or DQ8 gene variants, gluten exposure triggers your immune system to build antibodies against an enzyme called tissue transglutaminase 2, which lives in and around your intestinal cells. Attack the enzyme, and you damage the cells around it — over time flattening the villi that absorb your nutrients. That's a real, measurable, well-characterized disease: confirmed bloodwork, confirmed biopsy findings, a defined genetic risk profile. It is not up for debate.

Non-celiac wheat sensitivity is a different animal, and a November 2025 review in The Lancet shook up how we should think about it. Pooling controlled-trial data, the researchers found that a meaningful chunk of people who believe they're gluten sensitive react to gluten, wheat, and a placebo about the same — and proposed that a lot of what gets labeled "gluten sensitivity" is actually driven by FODMAPs (the fermentable carbs behind a lot of IBS and SIBO reactivity) or by a gut-brain feedback loop where expecting a reaction shapes the experience of one. Roughly 10% of the global population identifies as gluten sensitive; roughly 1% actually has celiac disease.

I want to be careful here, because it would be easy to hear that and think "so it's all in my head." That is not what the research says. Real, physiological, non-celiac gluten and wheat reactivity absolutely exists — the ATI mechanism above is proof of that on its own. What this tells us is that "gluten sensitivity" has become a catch-all label for several genuinely different things: a direct immune reaction to gluten, a reaction to non-gluten proteins like ATIs and serpins, a gut irritated by FODMAPs that would react just as much to onions and garlic, and a real gut-brain sensitivity pattern where anxiety about the food is part of the physiology, not separate from it. Lump all of that into one bucket, and you'll never fix the right thing.

Does Gluten Actually Change Blood Flow to Your Brain?

This one is real, but it's older than most of the internet content repeating it. Back in 2004, researchers in Rome used a brain imaging technique called SPECT to scan blood flow in 45 people — some with untreated celiac disease, some who'd been gluten-free for about a year, and healthy controls. The result: 73% of the untreated celiac group had at least one region of reduced blood flow to the brain, most often the frontal cortex — the part responsible for focus, planning, and executive function. Among celiacs already gluten-free, that number dropped to 7%. That's part of why brain fog shows up so often as a celiac symptom, not just a gut symptom.

Here's the honesty check: that data is specific to confirmed celiac disease. It is not proof that everyone with a vague gluten sensitivity has reduced blood flow to their frontal cortex. A newer framework — the gut-brain-heart axis, described in an April 2026 review in Brain & Heart — proposes that leaky gut drives systemic inflammation, which stresses your blood-brain barrier and injures the small vessels feeding your brain. Plausible, but the authors are upfront that real gaps remain in exactly how it works.

Does Gluten Really Cross-React With Your Thyroid?

This is the question I get asked more than almost any other by patients already managing Hashimoto's, and it's the one where the nuance matters more than the headline. A May 2026 review in Frontiers in Endocrinology laid out four candidate mechanisms linking gluten to autoimmune thyroid disease: gut dysbiosis and barrier dysfunction bridging inflammation from your gut to your thyroid; immune cross-reactivity — your immune system mistaking a piece of gluten for a piece of thyroid tissue because the two look similar at the molecular level, the actual "molecular mimicry" idea; shared epigenetic patterns between celiac and thyroid autoimmunity; and general immune spillover from a leaky gut.

The authors' own conclusion is the sentence worth remembering: these processes may converge to facilitate thyroid autoimmunity, but direct evidence that gluten-specific immune responses routinely target thyroid tissue is still limited. Cross-reactivity between gluten and your thyroid is biologically plausible. It is not proven. Those are two very different scientific claims, and blurring them helps no one.

Let me say that last part again, because it's the whole point of this section: gluten opens your gut a little in everyone, and a lot more in celiac disease. It's tied to real, measurable brain blood flow changes — but only in confirmed celiac disease. And its relationship with your thyroid is real enough to investigate, but not proven enough to treat as settled fact. Different mechanisms, different strength of evidence — but that doesn't mean gluten gets a pass. Once gluten or ATI-driven inflammation fires off in a genetically primed person, that alone can trigger Hashimoto's, whether or not gluten itself ever touches thyroid tissue directly.

How Do You Actually Test for This Instead of Guessing?

Most people get the order backwards. They go gluten-free first, feel a little better (or don't), and call that their diagnosis. Here's the problem: cut gluten before you're tested, and you can make every antibody test in this section come back falsely negative, because your immune system stops producing the antibodies it only makes when gluten is actually in your system — and that takes about six weeks to clear. Test first. Eliminate second. Reverse the order, and you've ruined your own data.

Think of the standard celiac panel like a smoke detector in one room of your house. It'll catch a fire in that specific room every time. It tells you nothing about the other five rooms. Cyrex's Wheat Burden Test and Vibrant's Wheat panel are more like a whole-house detection system — more sensors, more rooms covered, catching what the single detector was never built to see. That doesn't mean everyone needs the whole-house system. It means that when the one-room detector comes back clear and you're still clearly smelling smoke somewhere in the house, you don't shrug and assume you imagined it. You bring in better instruments.

THE FINAL WORD

Is gluten really that bad for you? It depends. If you're one of the lucky ones who can tolerate the transient leaky gut that affects everyone, then no — at least not yet. The moment your blood sugar destabilizes, your gut gets wonky, your hormones drift, or you're already inflamed, that same gluten you've tolerated for years suddenly becomes part of the problem.

You cannot sort out where you land on this spectrum by how you feel after a simple elimination diet. You have to actually test for it, in the right order, with the right panel for your specific clinical picture. That's the exact kind of untangling I do with people in my practice every week — patients who've been gluten-free for three years and still don't know if they're gluten sensitive or have celiac disease, and patients with Hashimoto's who were told to cut gluten with no explanation why, and no thyroid antibody testing before or after.
